This version has yellow "Duck Records" labels that read "Limited Edition Collectors Record".
The titles printed on the labels are all fake, like "Smoke on the duck", "I wanna hold your duck", etc.

Released in a white neutral sleeve with an 8 1/2 x 11 sheet of light blue paper and black print pasted onto it. Spine and back of the sleeve are blank. The sleeve lists the actual track titles.

"☮ means [i. e. track A1] it's the Amboy Dukes at the Woodstock Music Festival → special thanks to our friend and hers, the Wizard (Oh!). The rest was recorded at The Swing Auditorium (Ted'll tell ya the city) on May 1, 1976 [...]"
