The original sleeve was meant to be an eight-page booklet. The booklet idea was abandoned due to cost issues, with only a prototype sleeve having been made. The front artwork remained the same and the photo on the back remained as well, but the test pressings were housed in sleeves comprised of the original outer sleeve that would have been part of the booklet, had it been produced.

Recorded live at the Phoenix Theatre in Petaluma, CA on December 29th, 1993.
