USA radio station issue, with 'smoke logo' & 'Straight' black liner bearing the 'Bizarre' address - which is also on the radius text of the 'Straight' vinyl labels.
This is a Straight issue, under the short-lived Bizarre Inc distribution aegis of Reprise.

"Reprise Records Radio Station Service" sticker on front cover with track and time listings.
"Not For Resale" on sticker.

White label "Promotion Not For Resale" printed on label. Label maintains the Straight Records address on Wilshire Blvd. The label arranges the time listings of each track separate from the track listing and also includes a total playing time for each side.

Unknown if the promo copy was shipped with the lyric sheet insert. Black inner sleeve is present.

This version pressed at Columbia Records Pressing Plant, Santa Maria, as indicated by the 'S' in the runouts.
