The screen-printed folder and hand-numbered insert were available to those who purchase during a special "early bird" sale.

There are some typographical errors on the foldover sleeves.
"The Death Of Halpin Fraser" is identified as Themes From Hell 16 on the foldover sleeve, but Themes From Hell 15 on the labels.
"The Great God Pan" is identified as Themes From Hell 17 on the foldover sleeve, but Themes From Hell 16 on the labels.
"Count Magnus" is identified as Themes From Hell 15 on the foldover sleeve, but Themes From Hell 17 on the labels.
As the numbering sequence on the labels matches the sequence of the catalog numbers, the numbering on the labels has been followed in the tracklisting above.
