Two-piece sleeve in 7"x10" plastic bag. Front is a photo of two of the band-members ["Rich does not appear on the cover because he went to the beach instead."], stenciled with the band name and spray painted.
Back is folded 7" x 11" with lyrics and credits.

Two versions of the front sleeve stencil exist: one stencil has relatively even spacing but the letters vary up-and-down [this]. The other stencil [Chronic Disorder - Chronic Disorder EP] has the letters C & H larger and the letters line up somewhat more horizontally. Both versions often show an overspray area at the top where the stencils were placed too low.
The printed insert is identical for both versions.

Two versions of the back 7"x11" insert exist with this sleeve. One is printed (or a high-quality photocopy) in solid black and trimmed so that no lyrics or credits are cut off, the other is a typically grey photocopy, hand trimmed to 7" wide sometimes missing a line or two. Both versions occur with this version of the sleeve.
