Sky Fi is a now defunct label based in the San Francisco area.
There's a second sleeve version of this with a plain die cut sleeve with the band name stamped on one side. This came in a stapled plastic sleeve with a thin card price label attached. All the copies I've seen have the same 4 digit number on the price label.
