In plastic 9" wallet with backing 9" X 9" paper with a band biography, credits & the guitarist's address & phone number. The biography paper is invisibly glued to the back plastic, which is good & should not be taken out of the plastic or ink will run.

1 song 1-sided version with plain white label with hand written title (possibly test press & writing probably by member since also has band stamp). Non-play side is the "picture side" which appears same as the 1000 press 3 song version. This 1 song was probably released first as an advance promotional since all other versions are 3 songs & 2-sided.

Label number can only be found by the etching (not on the insert sheet).
