Some - or all - copies are mislabeled and include a small insert explaining the error.

On this version the colors of the label are reversed & the printing of the insert is of better quality, not a photocopy. .. For this reason I think it predates the other version : Generic, Mortal Terror - Generic / Mortal Terror
