The front and back of the cover is a silkscreen brown grocery store bag. The rear image of a ship shows no words or an address like other versions. It contains only one double sided foldout insert. On the back of this insert, the P.O. Box number (81332) within the address differs from most of the other versions (see photo). Side A: "GRAVITY 1." Side B: "2." The matrix on both side A and B are identical to the first press.
The labels are silver instead of black.
