Catalog number "LPM 3766" on sleeve, "LPM 3766" on labels.
Issued with RCA company inner sleeve offering a "Complete RCA Victor Catalog 25¢" with RCA's Rockaway address. These have date-codes ["RCA 21-112-1-43F" on this example] but all of the LP titles displayed were already a couple of years old.
