Each of tracks A2 - A4 and B1- B4 is effectively two tracks in one: a short version, followed by a long version. These tracks are separated on the actual vinyl, but the label only describes one track (see images).

My copy was found in a charity shop, sleeveless, but Popsike gives details of a copy with gatefold sleeve, and booklet. (See http://www.popsike.com/php/detaildatar.php?itemnr=2597922802)

The 'booklet' is actually more of a sheath with just the covers being stapled and the music being on individual sheets of paper (for easier use on a music stand presumably). There is no sheet music for the first track.
