The handwriting on the labels was inscribed by the acetate's owner, who received the disc from the artist in 1991. The current images are showing the labels from the 2014 vinyl reproduction (The Darkside), which was made from recordings and photos of this acetate; they're basically accurate, other than the pressing rings in the middle and the vinyl's runout etchings visible around the edges.

Track A was released in 1992 as: Longer Than Long Ago - A Relic.
Track B was released in 2009 on: Music Institute 20th Anniversary (Pt 1 Of 3).
