This version has the original withdrawn sleeve on the 4th EP. A small quantity of boxsets containing the sleeve were distributed to band members and record company employees before the official release. The sleeve was ultimately deemed inappropriate and all copies were removed from the boxset and destroyed. The 4th EP was then re-sleeved with an alternate cover before the boxset was released to the general public on December 14, 2010.

This version credits Danny Goldberg, Brady Brock, and Gold Village Entertainment as management, unlike later versions that credit Dimitri Coats as management.

Includes booklet with liner notes, credits, and lyrics. Includes download code for all four EP's on a hand-cut piece of paper.
