Released 1978 as a limited edition of 100 2LP box-sets by Static Music Ltd.

Recorded at All Souls Unitarian Church, Kansas City, Missouri, on February 16, 1978.

Black box with removable lid includes cover photo (a skeleton hand playing a piano) pasted on front lid, solarized brick wall photo of the church pasted on to inside of front lid and a loose photo numbered on the back out of 123 (not 100 like the box set). There is a track-listing sheet with information about the release and indicating it was a limited edition of 100 copies, followed by a handwritten copy number. At least one copy has a type-written label stuck on it that says " NB: Apologies for surface noise & clicking esp. on side C. This is due to pressing faults." (see images). While all known copies have defects, they are not confined to Side C.
There is a large foldout posters with 3 drawings of a hand strumming a guitar. In order, they show the hand as a skeleton, a series of muscles and a flesh-covered hand.
