The rear sleeve is black and contains the following texts in white : artist name, track title, Columbia and Sony logos, 'For Promotional Use Only - Not For Resale' plus copyright and manufacturing verbiage.

Came in a clear plastic wallet with a publicity sticker on it (see scans)
