All copies were supposed to have a fold-up paper insert that includes all lyrics, extended thanks list for each member and for Underground Express, a Severe Warning Merchandise order form, and other band info and credits. However, many copies were sent out before the completed inserts were added, so while many copies still include the insert many copies are missing it as well.
