Issued in a hand-glued picture sleeve with two folded inserts: one is a "Boycott Fascist Culture" manifesto with a "Why Rock Destroys Your Mind" flyer on the other side, and the second is a T-shirt offer that exists in at least two versions:
• The earlier version has two shirts offered and has a collage of letters and press-clippings/reviews printed on the back.
* The second version adds a third T-shirt design (using a Nosferatu image) and advertises "Up and coming bands on Ruthless" - Trial By Fire and Naked Raygun - and is blank on the back. Some (all?) show the "Haunted Town" price crossed out as "Sold Out / not available", and add "Also coming: Big Black 'Lungs'". See Images.

Ruthless' original street address (319 S.Jefferson, Chicago, IL 60606) is printed on the back sleeve, but it appears that they were already using the P.O. Box 1458, Evanston, IL 60204 address by the release-date. All known inserts show the new address, and it may be updated with (a) a rubber-stamped generic white sticker on the back sleeve, (b) on the plain inner-sleeve - using the same rubber-stamp and no sticker on the back, or (c) a black-bordered printed address sticker on the back sleeve. Some early copies have random/cryptic inscriptions written inside the hand-assembled sleeve.

Lyrics to both songs are printed on the "Question" side label.
