With 4-page booklet. Some copies have barcode sticker on cd case. Some copies may also have Dick's Picks 1-18 catalog.

The first disc on some copies had a manufacturing defect causing a "pop" during "Queen Jane" at approx. 1 minute 10 ten seconds into the tune. Replacement discs were enclosed with orders from the Grateful Dead Mercantile.
