Released in November of 2015, this lathe-cut single is limited to 14 copies and was available exclusively from Paper + Plastick's online store for $15 USD. It came in a poly-lined plain white die-cut paper sleeve that was embossed with the phrase "Everybody's Addicted To Something" with the words "GNV•FLA" above and a P+P Skull Logo beneath it. The band's name and the edition number is handwritten on the opposite side of the sleeve.

All credits are uncredited.

All matrix codes were individually cut, and it appears that they varied from one disc to the next (possibly in an attempt to match the number on the sleeve, though the copy in the pictures fails to do that).
