The original issue of this Cook binaural sampler. The cover is constructed of a trimmed, yellow hardboard square containing the cover graphics, which was then pasted on a blank white 10" cardboard cover. The rear of the cover is left blank.

The label describes the A-side contents as "The A B C's OF BINAURAL, as taught by Prof. TITUS MOODY" "A BINAURAL TRIP TO THE BIG CITY IN 3 DIMENSIONS"... the actual contents are listed on the cover, and are detailed here. The B-side consists of excerpts from various Cook Laboratories releases.

At some point, a proper cover was designed for this sampler, and the graphic shown here was used for the rear cover graphics.
