Release has four black labeled sides, with white text indicating catalog number and side.
This release does not feature the side A locked runout groove of the original.
Inside the sleeve is a fullsize print with the double iris eye on one side, and a 35mm filmstrip print
with the tracklist and photos of the band.

Record 1 is pink & record 2 is red. Both are see through. The 12"X12" 'eye image' is an insert, but the 'doctor & screaming patient' and 'standing man' are not.

This jacket has smaller "TOOL AENIMA" and "Zoo" logos. It also has the "BMG" logo between the "Zoo" logo and the barcode.
This release does not appear to have any catalog number on the spine.
